Optimising Mobility

Falls and poor mobility are very common problems for patients who have a life limiting condition. Falls can be disabling, frightening and distressing for all those concerned. This leaflet will explain how the Optimising Mobility & Falls Prevention course can help.

Who is the course for?

The Optimising Mobility & Falls Prevention course is for patients who are troubled by falls, for patients who are at risk of a fall, and for patients who may benefit from a self-help programme in order to manage their poor mobility.

What does it involve?

We will continue to support patients with mobility with one to one telephone appointments or virtual one to one meetings. Support with using technology will be provided too if required. Community visits are also possible where the patient requires this instead of telephone or virtual support.
